Infants and children: Oral Dose: Initial: 0.5-2 mg/kg/dose PO Q6H - Q24H Maximum PO dose 4 mg/kg/dose Parenteral: 0.5-2 mg/kg/dose /IV Q6-24H PRN. Usual: 1 mg/kg/dose IV Q6-24H Maximum single dose is 2 mg/kg/dose IV. Cont. IV infusion in Critical Care areas 0.1-0.5 mg/kg/hr Note: older children and adolescents often respond to smaller doses (10-20 mg/dose) Adults: Oral dose:20-80 mg/dose PO Q6-24H. Maximum 600 mg/24 hr Parenteral: 10-40 mg/dose IV Q6-12H. Usual maximum: 80 mg/dose IV |
May cause hypokalemia, alkalosis, dehydration, hyperuricemia, increased calcium and magnesium excretion, nephrocalcinosis. Ototoxicity may be associated with large IV doses and rapid injection especially in patients with pre-existing renal impairment or taking other ototoxic drugs. Parenteral doses should be infused slowly over 12 min. or given at a rate not to exceed 0.5 mg/kg/min. Higher doses may be required in patients with renal failure; to initiate a response. * Oral Biovailability is poor, 45-65%.
