Bone Marrow Transplant: 1.5-3 mg/kg/dose IV Q12H. 5-10 mg/kg/dose PO Q12H. 2-6 mg/kg/dose PO Q12H Focal Segmental Glomerulosclerosis (FSGS): Initial: 1.5 mg/kg/dose PO Q12H Initial: 1.25 mg/kg/dose PO Q12H Increment: increase by 0.5-0.75 mg/kg/24 hr if insufficient response after 8 weeks of treatment.Maximum: 4 mg/kg/24 hr PO Autoimmune disease: 0.5-1.5 mg/kg/dose PO Q12H 1 mg IV = 3 mg PO; recheck serum level in 4-8 days. |
These doses are guidelines only. Due to the erratic absorption of cyclosporine, and the increased clearance rate of cyclosporine in infants and young children, cyclosporine levels should be monitored regularly to ensure that a therapeutic range is maintained. Oral solution is supplied with just ONE oral syringe for measuring dose; please do not discard this syringe. Any syringe can be used for measuring doses: use same syringe every day; don't wash it - wipe off after use; discard the very first dose drawn up with the syringe (to allow binding to become "saturated"). See the CPS for details on administering the oral liquid. G-tube: instill through extension in g-tube button; flush well with 10 mL water, apple or orange juice.
