DIPYRIDAMOLE (Paeds)
(Persantine)

Mechanism of action:

Antiplatelet; vasodilator.

Dipyridamole:

1) inhibits the phosphodiesterase enzymes that normally break down cAMP (increasing cellular cAMP levels and blocking the platelet aggregation response to ADP) and/or cGMP.

2) inhibits the cellular reuptake of adenosine into platelets, red blood cells, and endothelial cells, leading to increased extracellular concentrations of adenosine.

Children:
2-5 mg/kg/24 hr PO div. TID.

Post-op Ventricular Assist Device (Berlin Heart):
1 mg/kg/dose PO Q6H
ASA alternative.

Variable oral absorption; to optimize take on an empty stomach (1 hour before or 2 hours after meals)

Has peripheral vasodilator effects: use with caution in patients with hypotension

In patients with Berlin Hearts dipyridamole is used in combination with ASA as part of Berlin Heart anticoagulation protocol - see Berlin Heart on Team site at BCCH
