Tablet:  50 mg, 100 mg
Liquid (BCCH): 20 mg/mL
Infants/children:
Initial:  16-35 mg/m2/dose PO Q8H
Usual:  35-50 mg/m2/dose PO Q8H
Maximum: 200 mg/m2/24 hr
OR
Initial: 0.3-1 mg/kg/dose PO Q8H
Usual: 1-2 mg/kg/dose PO Q8H
Maximum: 8 mg/kg/24 hr
(consider BID dosing in older children)

Amiodarone increases flecainide levels; decrease flecainide dose by 30-50% if used in combination with amiodarone (CYP450 2D6 substrate).

Use with caution in presence of CHD; may aggravate LV failure, sinus bradycardia and pre-existing ventricular arrhythmias.

Monitor QRS duration, increased PR interval.

Dairy products (milk, infant formula) may decrease absorption of flecainide.  Avoid concurrent administration with formula feeds.

Reduce dose in renal failure.
