Bioavailability >90%. May cause QT prolongation. Caution in patients with proarrythmic condition or renal and hepatic dysfunction. Use with caution if on other QT prolonging medications. Dosage adjustment required in renal impairment. Good CSF and vitreous penetration; achieving concentrations of > 50% of those in serum. Affects CYP450 resulting in multiple drug interactions. May increase effects/levels of cyclosporine, phenytoin, vincristine, warfarin and zidovudine. Consult pharmacy prior to starting any drug therapy. For serious invasive fungal disease, consider a loading dose of 12 mg/kg/dose PO/IV x 1 for pediatric patients, or 800 mg PO/IV x 1 for adults.
