Water soluble B vitamin (B9) supplement

tablet: 1 mg, 5 mg
injection: 5 mg/mL (10 mL)
oral liquid (BCCH): 1 mg/mL
FOLIC ACID DEFICIENCY:
Infant/Child/Adolescent:
Initial:
0.5-1 mg PO/IV/IM/SC daily for 3-4 weeks, until laboratory evidence of hematologic recovery
Maintenance:
Infant:  0.1 mg/day PO/IV/IM/SC
< 4 yrs old:  up to 0.3 mg/day PO/IV/IM/SC
> 4 yrs old:  0.4 mg/day PO/IV/IM/SC

Note: Multivitamin containing 0.2 mg folic acid may be adequate for maintenance.

Adult:
1-5 mg PO daily

PROPHYLAXIS FOR SICKLE CELL DISEASE:
1 mg PO daily

Will not prevent neurologic abnormalities due to vitamin B 12 deficiency, although the hematological effects may be masked.

IV form can be given orally.


Standard Prescription:

Infants: folic acid__mcg PO/IV/IM/SC Q__H (__mcg/kg/dose)

>1 yr old: folic acid__mcg PO/IV/IM/SC Q__H (__mcg/24hr)
