1 and 2 mL amp., 1, 5, 10 mL vials
25,000 units/500 mL D5W bag (50 U/mL)
Pediatrics:
Loading Dose: 75 units/kg IV bolus (max: 5000 units)
Initial Maintenance (continuous IV infusion):
<= 1 year old: 28 units/kg/hr
>1 year old: 20 units/kg/hr
"Low dose" 10-15 U/kg/hr

Adolescent and adult:
18 units/kg/hr (max: 1000 units/hr)
Adjust dose to maintain APTT in range of 1.5 to 2.5 times control (60-85 sec).

Major toxicities are bleeding, allergy, thrombocytopenia.
