LEUCOVORIN (Paeds)

Mechanism of action:

Folic acid derivative; supplies the necessary cofactor blocked by methotrexate

Rescue Dose After High-Dose Methotrexate (MTX):
10 - 15 mg/m 2 /dose PO/IV Q6H x 72 hr.

Doses as high as 1000 mg/m2/dose may be required when there is delayed methotrexate elimination.  Dose and frequency are then guided by serum methotrexate levels

Refer to individual chemotherapy protocol for dose and duration.

Due to saturable absorption, doses larger than 25 mg should be administered IV.

Leucovorin is normally given until serum methotrexate level < 0.1 umol/L.

Due to calcium content, IV infusion rate is not to exceed 160 mg/min.
