Rescue Dose After High-Dose Methotrexate (MTX): 10 - 15 mg/m 2 /dose PO/IV Q6H x 72 hr. Doses as high as 1000 mg/m2/dose may be required when there is delayed methotrexate elimination. Dose and frequency are then guided by serum methotrexate levels |
Refer to individual chemotherapy protocol for dose and duration. Due to saturable absorption, doses larger than 25 mg should be administered IV. Leucovorin is normally given until serum methotrexate level < 0.1 umol/L. Due to calcium content, IV infusion rate is not to exceed 160 mg/min.
