injection:
1% (10 mg/mL) 5 mL amp., 20 mL vial.
2% (20 mg/mL) PLS, 5 mL amp., 20 mL vial.
20% (200 mg/ mL) PLS 1 g/250 mL (4 mg/mL) D5W bag.
Antiarrhythmic:
Loading Dose:
IV/IO:
1 mg/kg/dose over at least 2 min., then begin continuous infusion. If > 15 min. delay in starting infusion, give a second dose of 0.5-1 mg/kg/dose. Maximum 100 mg/dose or 5 mg/kg total dose.
ETT: 2-3 x IV dose.

Continuous Infusion:
IV/IO:
20-50 mcg/kg/min.
Maximum infusion rate: 4 mg/min.

For patients with CHF, hepatic disease or shock: Infusion rate < 20 mcg/kg/min.

Local anesthesia:
1% lidocaine 0.3 to 1.5 mL intradermal for PICC insertion

For peridural or regional anesthesia, dose varies with procedure, tissue vascularity, degree and duration of anesthesia required.

Maximum 4.5 mg/kg/dose, not to exceed 300 mg/dose
Administer loading doses slowly, over at least 2 min.

Follow endotracheal route (ETT) administration with a 5 mL NS flush.

Administer with caution in patients with liver disease, CHF, heart block, sinus bradycardia.

Contraindicated in complete heart block and wide complex tachycardia due to accessory conduction pathway.

Excessive doses or impaired clearance results in myocardial depression, hypotension, central excitation and seizures.

With prolonged use, monitor to maintain drug levels in therapeutic range of 7-29 micromol/L (1.5-6 mg/L).

Caution: lidocaine products containing preservatives or epinephrine are not intended for IV use.
