Antibiotic; amebicide; antiprotozoal

Children:
5-15 mg/kg/dose  PO/IV Q8H
Max. 500 mg/dose

Perforated Appendicitis:
IV: 30 mg/kg/dose IV Q24H
Max. 1000 mg/dose

PO: 10 mg/kg/dose PO Q8H
Max: 500 mg/dose

Crohn's Disease
10 mg/kg/dose PO BID to TID. Max. 500 mg/dose

Clostridium difficile:  
Children: Mild to moderate infection, first or second episode: 7.5 mg/kg/dose PO QID x 10 days
Severe complicated infection, with vancomycin: 10 mg/kg/dose IV Q8H x 10 days
Max. 500 mg/dose

Vaginal Therapy:  
One applicatorfull PV nightly x 10 days

Gastrointestinal tract decontamination in intestinal failure:
10 mg/kg/dose PO BID x 1-2 weeks

Adults:
500 mg PO/IV BID to TID
Excellent oral bioavailability of 100%.

Avoid alcohol containing preparations for 24 hr after dose to prevent disulfiram-like reactions.

Once daily IV dosing used only for perioperative period until able to tolerate oral regimen
