Inotrope/Vasodilator: Loading dose: 50 mcg/kg/dose IV over 10 min. Maintenance: 0.25-1 mcg/kg/min by continuous IV infusion. *NB: Loading doses in post-op open heart patients require approval from a pediatric intensive care physician. |
Bipyridine phosphodiesterase III inhibitor resulting in positive inotrope effects and vasodilatation. Contraindicated in obstructive cardiac lesions such as pulmonary and aortic stenosis and coarctation of the aorta. Side effects include: headache, dysrhythmias, hypotension, hypokalemia, abnormal liver function tests, tremor, thrombocytopenia (lower incidence than amrinone), bronchospasm. Half-life is about 2 hrs in children with normal renal function. Half-life is prolonged in patients with renal dysfunction.
