Delayed release tablet: 10 mg, 20 mg

Delayed release capsule: 20 mg (Note: 10 mg and 40 mg strengths are commercially available, but not on formulary)

Oral suspension (BCCH): 5 mg/mL in 8.4% sodium bicarbonate (provides 1 mEq/mL of Na+ ) [use with caution in cardiac patients]
Children:

GERD, esophagitis, ulcers:
1 mg/kg/dose PO once daily (0.7-3.5 mg/kg/day).
Maximum: 40 mg/day

H. pylori eradication (as part of triple therapy regimen):
0.5-1 mg/kg/dose PO BID (Max: 20 mg PO BID) for 7 to 14 days.
Oral suspension can be given through G, Jand NG tubes.

Optimal dose administration is once daily, before the first meal of the day. If twice daily dosing is needed, administer all doses before meals.

Children 1 to 10 years old may require higher doses due to increased clearance.

Delayed release tablets and capsules should be swallowed whole (do not crush or chew).

Potential for drug interactions (CYP 2C19 substrate and inhibitor, effects on gastric acidity): may decrease clearance of diazepam, phenytoin, and warfarin; may decrease absorption of itraconazole, ketoconazole, iron salts; may increase serum levels of tacrolimus.

Requires Special Authority from Pharmacare for out-patient prescriptions. (only GI physicians may prescribe omeprazole without Special Authority).
