ALFACALCIDOL (Paeds)
(1-alpha-hydroxyvitamin D3, One-Alpha)

Mechanism of action:

A synthetic Vitamin D metabolite which requires hepatic activation; does not require renal activation.

capsule:  0.25 mcg
oral drops: 
2 mcg/mL (1 drop = 0.1 mcg). 10 mL bottle

*Use drops within 28 days of first opening bottle*

Children:  

0.01-0.05 mcg/kg/dose PO once daily.

Increase dose every 4 to 8 weeks based on clinical response.

Usual maintenance dose in children is 0.25-1 mcg once daily.

Usual adult maximum: 1-2 mcg once daily

Dose adjustment based on serum PTH.

Usual maximum doses may be exceeded in consultation with nephrology or endocrinology

Monitor calcium, phosphate, iPTH, alkaline phosphatase during dose titration.
