Haematology Consult is required for use Direct Thrombin inhibitor for Heparin induced thrombocytopenia Target PTT 1.5-3 x normal range, check PTT 2hr post bolus Risk of bleeding, hypersensitivity discontinue other parenteral anticoagulants prior to initiation caution if increased risk of bleeding (post LP, recent surgery, uncontrolled hypertension) avoid NSAIDs or other antiplatelet agents Standard Prescription: argatroban __mcg (__mcg/kg) IV x 1 dose (loading dose) argatroban __mcg/kg/min continuous IV infusion |
