Children: JET: IV (loading dose): 3-6 mg/kg/dose over 20 mins. Maximum: 100 mg/dose. May repeat every 5-10 mins to a maximum of 15 mg/kg. IV Maintenance: 20-80 mcg/kg/min. by continuous IV infusion. Maximum: 80 mcg/kg/min or 2 g/24 hr or 6 mg/min. SVT: 10-15 mg/kg//dose IV/IO over 30 min (Maximum 1000 mg/dose) Challenge for Brugada syndrome: 15 mg/kg/dose up to a maximum of 1000 mg over 20 minutes |
Class Ia antiarrhythmic with anticholinergic and local anesthetic and negative inotropic effects May cause lupus-like syndrome. Given IV may cause hypotension, AV block, widening of QRS. Widening of QRS > 0.02 sec. suggests toxicity. Short half life requires frequent dosing.
