ZUCLOPENTHIXOL (Paeds)
(Clopixol-Acuphase)

Mechanism of action:

Typical antipsychotic, antagonises both dopamine D1 and D2 receptors, α1-adrenoceptors and 5-HT2 receptors.

Acuphase injection: 50 mg/mL (2 mL ampoule)

Adolescents/Adults:
Usual: 50-100 mg IM every 2-3 days PRN
Maximum total cumulative dose (within 30 day period): 400 mg and/or 4 injections.
Acuphase injection is intermediate acting (sedation onset within 2 hrs, peaks at 8 hrs. Peak serum level occurs in 24-48 hrs; Note: 48-72 hrs duration of action).

A second injection may be required, 2 to 3 days after the first. Acuphase formulation is not intended for long-term use.

For severe/repeated aggression or agitation in patients unwilling to take oral medication.

Acuphase formulation should not be administered to antipsychotic-naïve patients or intoxicated patients.

May cause QTc interval prolongation.

Sedation common with initial dose, tolerance to this effect may develop with repeated doses.

Major CYP 2D6 substrate. May have prolonged duration of action in patients who are CYP 2D6 poor metabolizers, or patients taking strong CYP 2D6 inhibitors.

For injection volumes greater than 2 mL, the dosage should be split and administered at two separate sites (gluteal).
