Pre-op (inhibit secretions): Infants & Children < 5 kg: 0.02 mg/kg/dose SC/IV/IM/PO. Maximum: 0.6 mg/dose Infants & Children > 5 kg: 0.01-0.02 mg/kg/dose SC/IV/IM/PO. Maximum: 0.4 mg/dose Adults: 0.5 mg/dose IM/IV/SC CPR (bradycardia): 0.02 mg/kg/dose IV/IO Q5 min x 2 doses PRN. Max single dose 0.5 mg Max total dose: Children: 0.5 mg x 2 = 1mg Adolescents and Adults: 0.5mg x6 = 3 mg 0.04-0.06 mg/kg via ETT. Dilute with NS to a volume of 3-5 mL and follow with several positive pressure ventilations. Organophosphate Poisoning: Consult Drug and Poison Centre Children <12 years: 0.05 mg/kg/dose IV/IO initially; then doubling the dose every 5 min. until control of excessive bronchial secretions, and correction of significant bradycardia and hypotension (i.e. resolution of muscarinic symptoms). Children >=12 years: 1 mg/dose IV/IO initially; then repeated by doubling the dose every 5 min. until muscarinic symptoms reverse |
IV injection is given over 1 minute. For endotracheal administration use higher doses (2-10 X IV dose). Dilute with NS to a volume of 3-5 mL and follow with several positive pressure ventilations. Injectable form may be given orally and is well absorbed; peak plasma concentrations are achieved within 1 hour. Toxic Anticholinergic Side Effects: Red as a beet (skin flushing), Dry as a bone (dry mouth, urinary retention), Blind as a bat (blurred vision, photophobia), Mad as a hatter (agitation), Hot as a stove (anhidrosis). Standard Prescription: atropine __mg IV/IM/SC/PO/ETT x__dose(s){PRN} (__mg/kg/dose) |
