May cause diarrhea and worsen malabsorption by decreasing bile salt pool. Aluminum-based antacids (Almagel, Amphogel, Maalox) can bind to ursodiol and decrease its absorption; do not use antacids within 2 hours of ursodiol dose. Take with food. In patients with Primary Sclerosing Cholangitis, maximum dose is 20 mg/kg/day. Monitor liver enzymes and bilirubin while on therapy.
