Transplantation, solid organ: Initial: 3-5 mg/kg/24 hr IV or PO daily beginning at the time of transplant. Maintenance: 1-3 mg/kg/24 hr IV or PO daily. Inflammatory Bowel Disease: 1-3 mg/kg/day IV/ PO daily. Maximum daily dose 4 mg/kg/24 hours, or 200 mg per day. Doses higher than 2 mg/kg should be guided by thiopurine metabolite levels. |
Bone marrow suppressive. Monitor hematological status and liver function May also cause rash, stomatitis, GI disturbances. Reduce dose in renal impairment. allopurinol significantly interacts with azathioprine; reduce dose of azathioprine by 1/3 to 1/4 of usual dose Refer to Nursing Policies under "cytotoxic drugs" for safe handling, administration, etc. Standard Prescription: azathioprine __mg IV/PO daily (__mg/kg/24hr) |
