SPIRONOLACTONE (Paeds) (Aldactone) Mechanism of action: Potassium-sparing diuretic; selective aldosterone antagonist Ref: 29, 141, 192, 193 Last update: 2019-08-26	tablet: 25 mg, 100 mg oral suspension (BCCH): 5 mg/mL	Children: 1-1.5 mg/kg/dose PO once daily or BID Max Single dose: 50 mg Adults: 25-50 mg PO q12-24H Max adult dose 100 mg /day	Watch for hyperkalemia especially when combined with potassium supplementation or ACE inhibitors. Avoid use in severe renal failure (Creatinine Clearance <10 mL/min). Full diuretic and potassium-sparing effects occur several days into therapy due to accumulation of the active metabolite canrenone. Standard Prescription: spironolactone__mg PO Q__H
