Mechanism of action:

Class III antiarrhythmic agent (prolongs action potential) with minor class II (non-selective beta-blocking) effects.

Suspension (BCCH): 5 mg/mL
< 2 yr : 30 mg/m2/dose x age related  factor PO Q8H

Cardiology Consult required

>2 yr Initial: 0.7- 1 mg/kg/dose PO Q8H or Q12H
Usual starting dose: 1 mg/kg/dose PO Q12H
Increment: 1-2 mg/kg/24 hr every 3 days
Maximum: 2.7 mg/kg/dose PO Q8H (8 mg/kg/24 hours)
OR
Initial: 30 mg/m2/dose PO Q8H
Max: 60 mg/m2/dose PO Q8H

Adults: 80 mg PO BID to max 320 mg/24hr

Reduce dose in renal impairment.

Use caution in reactive airway disease.

May cause pro-arrhythmic effects e.g. prolongation of QT and risk of torsade, AV block.
