Secondary hyperparathyroidism in CKD:

Oral:  < 3 years: 0.01 mcg/kg/dose once daily.
>= 3 years: 0.25 to 0.5 mcg once daily  
(Maximum 1 mcg/day)

IV:  (use in hemodialysis)
0.25 - 0.5 micrograms IV 3 x weekly.  Titrate by 0.25-0.5 mcg Q2-4 weeks as necessary to a maximum of 4 mcg 3 x weekly.

Dose adjustment based on serum iPTH. Usual maximum doses may be exceeded in consultation with nephrology or endocrinology

Monitor calcium, phosphate, iPTH, magnesium, alkaline phosphatase frequently until dose is stabilized.

Injectable  may be used ORALLY. If liquid withdrawn from ampoule for storage in PO syringe prior to use, requires protection from light (wrap syringe in foil).

There are various dosing schedules depending on indication and further references should be consulted.

Does not require renal or hepatic activation.

Avoid oral use with mineral oil.
