CARVEDILOL (Paeds)


Mechanism of action:

Non-selective beta blocker with alpha-blocking activity.

tablet: 3.125 mg, 6.25 mg, 12.5 mg

oral suspension (BCCH):
0.1 mg/mL, 1.67 mg/mL
Children (CHF):
Initial: 0.03-0.08 mg/kg/dose PO BID (maximum initial dose 3.125 mg PO BID) Consider TID dosing in children <3.5 yr
Increment: Double dose every 2 weeks
Maintenance: 0.3-0.9 mg/kg/dose PO BID (Max 25 mg PO BID)

Adults:
Initial 3.125 mg PO BID Increment Double dose every 2-3 weeks.
To a maximum of 25 mg PO BID (<85kg) or 50 mg PO BID (> 85kg)
A non-selective beta blocker with alpha-blocking activity.

Allow 2-3 weeks between dose increments when treating CHF to avoid short term negative inotropic effects.

An adjunct to diuretics and ACE inhibitors (with or without digoxin) in management of CHF.

Carvedilol may cause a modest increase in serum digoxin level.

Due to faster elimination in young children (<3.5 yrs) TID dosing and higher dose may be required.

Poor oral biovailability 25-35% due to first pass effects.
