Seizures: Children > 2 years <=30 kg: Initial: 2.5-5 mg/dose PO daily Increment: 2.5-5 mg/dose PO daily every 5-7 days Maintenance: 0.5-1 mg/kg/dose PO daily (Divide doses > 20 mg to BID) Maximum: 30 mg/dose PO daily (In rare cases, may use higher maximum based on individual clinical response) > 30 kg: Initial: 5-10 mg/dose PO daily Increment: 5-10 mg/dose PO daily every 5-7 days Maintenance: 0.5-1 mg/kg/dose PO daily. Use lowest effective dose Patients who show no response to 1 mg/kg/24 hr are unlikely to respond to higher doses If daily dose is divided, the higher portion should be taken at night. CYP2C19 poor metabolizers may require slower titration Adverse effects: drooling, sedation, increased appetite, paradoxical reactions, behavioural changes (more likely in patients with ADHD or previous behavioural problem Standard Prescription: clobazam__mg PO Q__H (__mg/kg/24hr) |
