Standard Prescription

domperidone__mg PO__(frequency) (__mg/kg/24hr)

Dosages

GI Motility Disorders:
Children < 35 kg:
0.25 mg/kg/dose TID 15-30 min. before meals (*new dosing based on recent publications-see references*)

Adults and Adolescents >= 35 kg:
10 mg PO TID (Maximum: 30 mg/24 hr)

Mechanism of Action

Dopamine antagonist, Gastrointestinal prokinetic agent

oral suspension (BCCH): 1 mg/mL (for NICU only); 10 mg/mL

Comments

May cause elevated serum prolactin.  

Extrapyramidal reactions are rare and less frequent compared to metoclopramide due to poor penetration into CNS.

Due to increased risk for cardiac adverse effects (QTc prolongation, cardiac arrest and serious ventricular arrhythmias) do not use in children with co-existing cardiac disease, moderate or severe liver impairment, significant electrolyte disturbances, or are on QT-prolonging drugs.

Consider an ECG prior to initiation and during treatment for those at risk for QT prolongation.

CYP3A4 substrate with many possible drug interactions, check with a pharmacist.
