Dosages
Treatment of thrombosis:
<= 2 mo. of age: 1.5 mg/kg/dose SC Q12H
>2 mo. of age: 1 mg/kg/dose SC Q12H
Dose adjustment required in renal failure
Round to the nearest mg if possible
<= 2 mo. of age: 1.5 mg/kg/dose SC Q12H
>2 mo. of age: 1 mg/kg/dose SC Q12H
DVT prophylaxis:
reduce above dose by 50% or give 1 mg/kg/dose once daily
Dose adjustment required in renal failure

For doses less than 10 mg use 30 unit insulin syringe (1 U = 1 mg) to administer.
Store vials at room temperature; 28 day expiry once opened.
Hold 2 doses prior to any invasive procedure such as LP.
Requires "Special Authority" from Pharmacare if prescribing for home use.
Neonates may require higher dose.
