flumazenil__mg {IV or Intranasal} x 1 dose (__mg/kg/dose)
Dosages
Reversal of benzodiazepine sedation
Infants and Children:
Initial: 0.01 mg/kg/dose maximum 0.2 mg/dose) IV Q1 minute PRN to a maximum cumulative dose of 1 mg or 0.05 mg/kg, whichever is less.
Doses may be repeated in 20 min. up to a maximum of 3 mg in 1 hour.
If no IV access: 0.025 mg/kg/dose intranasal Q5 minutes until alert

Mechanism of Action
Antidote; nonspecific competitive antagonist of the benzodiazepine receptor.
Forms Supplied
injection: 0.1 mg/mL, 5 mL and 10 mL amp.
Comments
Onset of action: 1-3 min. Flumazenil has a short duration of action (less than 1 hour); repeat doses are often necessary to prevent resedation.
Patients must be closely monitored until all central benzodiazepine effects have subsided.
May precipitate seizures in patients with chronic benzodiazepine use or exposure.
Use with caution in liver dysfunction.
Administration via the intranasal route has limited evidence outside of the peri-procedural setting.
