Standard Prescription

Dosages

Children 3-12 years old (15-40 kg):
Initial: 0.01-0.02 mg/kg/dose PO BID-TID.
Increase total daily dosage by 0.25-0.5 mg/24 hr PO every 5-7 days PRN to a maximum of 0.15 mg/kg/24 hr.

Usual maintenance doses:

Agitation/Psychosis:
0.02-0.05 mg/kg/dose BID-TID. Suggested dose range: 1-4 mg/24 hr.

Tourette Syndrome:
0.25-0.5 mg PO BID. May increase total daily dose by 0.5 mg every 5-7 days.

IM dose for 6-12 yrs:
1-3 mg IM Q4-8H to a maximum dose of 0.15 mg/kg/24 hr

Adolescents (>40 kg):
Acute Agitation*:

2-5 mg IM or 1-10 mg PO. May repeat in 1 hr PRN. Usual maximum 10 mg/24 hr.

Psychosis:
2-5 mg IM Q4-8H or 0.5-5 mg PO BID. Usual maximum: 10 mg/24 hr.

Tourette Syndrome:
0.25-1 mg PO BID.

tablet: 0.5 mg, 1 mg, 2 mg, 5 mg (10 mg, 20 mg not stocked at BCCH).
injection: 5 mg/mL (1 mL amp).

Comments

Safety and efficacy have not been established in children < 3 years.

Very high incidence of extrapyramidal symptoms (EPS), including dystonic reactions in children and adolescents (Should always treat with prophylactic benztropine or diphenhydramine; consider prophylactic administration of anticholinergic agent or use of an alternate low-moderate potency antipsychotic (e.g. chlorpromazine, methotrimeprazine or loxapine).

Acutely agitated patients may require doses as often as Q60 min.

May cause hypotension (caution in cardiac disease), prolonged QTc interval, lower seizure threshold (caution in epilepsy).

2 mg = 100 mg chlorpromazine (approximately) in terms of clinical equivalency.

Many drug interactions via CYP3A4 inhibition.

* some highly agitated patients may require higher doses.
