Dosages
Infant < 6 mo.:
1-2 mg/kg/dose PO BID to max. 37.5 mg/day
Infant > 6 mo. & child:
1 mg/kg/dose PO BID to max 100 mg/day (child)
Adult:
25-100 mg/24 hr PO daily or divided BID to max. 200 mg/day
1-2 mg/kg/dose PO BID to max. 37.5 mg/day
Infant > 6 mo. & child:
1 mg/kg/dose PO BID to max 100 mg/day (child)
Adult:
25-100 mg/24 hr PO daily or divided BID to max. 200 mg/day

Thiazide diuretic

Comments
Monitor serum electrolytes; May cause hyponatremia, hypokalemia, hypomagnesemia.
Loss of efficacy at GFR<40 mL/min; avoid use if GFR <10 mL/min
Onset within 2 hrs; duration 6-12 hr
