piperacillin-tazobactam __mg IV Q6H (___mg/kg/24 hr)
Dosages
All doses are based on piperacillin component
Infants and Children:
75 mg/kg/dose IV of piperacillin Q6H. Max 4 g/dose
Greater than or equal to 27 kg: 100 mg/kg/dose IV of piperacillin Q4H (Max dose: 4 g/dose)
75 mg/kg/dose IV of piperacillin Q6H. Max 4 g/dose
Empiric therapy for fever & neutropenia:
75 mg/kg/dose IV of piperacillin Q6H (Max 4 g/dose)Cystic Fibrosis:
Up to 27 kg: 150 mg/kg/dose IV of piperacillin Q6H (Max dose: 4 g/dose)Greater than or equal to 27 kg: 100 mg/kg/dose IV of piperacillin Q4H (Max dose: 4 g/dose)
Adult:
3-4 g/dose IV Q6H
Mechanism of Action
Beta-lactam antibiotic with beta-lactamase inhibitor.
Forms Supplied
Injection: 4.5 g vials containing piperacillin 4 g and tazobactam 0.5 g
Comments
Tazobactam is a beta lactamase inhibitor. When combined with piperacillin its spectrum of activity is expanded to include beta lactamase producing bacteria (primarily S. aureus sp) that would otherwise be resistant to piperacillin by itself
Dose adjustment required in renal impairment.
Poor CNS penetration
