Standard Prescription

Dosages

IV:
50 mg/kg/dose IV bolus or infusion.
This may be followed by 50 mg/kg/dose by continuous infusion or div. Q4-6H. Maximum: 300 mg/kg/24 hr IV.

Oral:
16.7-33.3 mg/kg/dose PO Q8H.
Increase slowly to a maximum of 3 g/24 hr, if needed.

Doses up to 800 mg/kg/24 hr PO may be required in patients with excessive urinary loss.

Comments

Used for the treatment of carnitine deficiency due to inborn error of metabolism, end stage renal disease, valproate toxicity, and cardiomyopathy.

Oral doses should be given with or following meals to maximize tolerability.

Oral solution may be mixed in drinks or other liquid food.

May cause nausea, vomiting, abdominal cramps, diarrhea, body odor.
