Dosages
Children:
Traditional:
7.5 mg/kg/dose IV/IM Q8H
Maximum initial dose: 1500 mg/24 hr. Dose may be increased based on serum level
Extended interval:
15-20 mg/kg/dose IV Q24H
Cystic Fibrosis:
7.5-10 mg/kg/dose IV Q8H.
Maximum initial dose: 500 mg/dose. Dose may be increased based on serum level
Traditional:
7.5 mg/kg/dose IV/IM Q8H
Maximum initial dose: 1500 mg/24 hr. Dose may be increased based on serum level

Aminoglycoside antibiotic

Comments
Aminoglycoside antibiotic: ototoxic, nephrotoxic.
Adjust dose in renal impairment
Obtain baseline creatinine; for therapy beyond 7 days check creatinine at least twice weekly.
Dose may be adjusted based on serum concentrations.
Traditional dosing serum concentrations:
Peak (Post-dose): 20-35 mg/L;
Trough (Pre-dose): < 10 mg/L.
