Standard Prescription

Dosages

Agitation, Aggression, Psychosis:
Children (<12 yrs):
Initial:
0.125 mg/kg/dose PO BID Increase gradually as needed to control agitation/psychosis.
IM: 0.06-0.125 mg/kg/dose IM once daily or div. TID.
Maximum: 40 mg/24 hr.
Adolescents/Adults:
Initial: 2.5-10 mg PO BID-TID Increase gradually as needed to control agitation/psychosis. In severe cases may start with 25 mg PO BID-TID, IM: 25 mg IM BID-QID

Pain, Agitation, Delirium (Palliative Care):
0.05-0.2 mg/kg/dose PO/IV Q6-8H PRN. When titrating IV doses, may give Q30-60 min until stable dose is reached.
Maximum 0.5 mg/kg/dose (50 mg/dose)

Phenothiazine with analgesic and anxiolytic properties; typical antipsychotic

injection:  25 mg/mL (1 mL amp)

Comments

Dosage should be adjusted according to condition being treated and patient's individual requirements.

Excessive daytime sedation may be resolved by lowering daytime dose and increasing HS dose.

High incidence of sedative, anticholinergic effects; low-moderate extrapyramidal symptoms (EPS).

High incidence of orthostatic hypotension or reflex tachycardia  may be observed at start of therapy, following rapid dosage increase or following IV/IM administration.

Methotrimeprazine is used primarily for its sedating effects. May have lower risk for paradoxical excitation compared with benzodiazepines.

Oral bioavailability is 50%. Use caution with IM/IV doses (twice as potent as the same oral dose).
