< 18 kg: 0.5 mg PO Q12H.
18-30 kg: 1 mg PO Q12H.
>30 kg: 1 mg PO Q8H
Maximum: 0.06 mg/kg/day

Comments
May start with lower dose and increase as tolerated.
Not recommended for routine prevention of chemotherapy induced nausea and vomiting. When required, give the first dose the night before chemotherapy and continue for up to 48 hr after chemotherapy.
Generally reserved for use in older children and teenagers as younger children may find adverse effects intolerable. Adverse effects include drowsiness, dizziness, euphoria, mood changes.
