Dosages
Children:
Treatment: 1.25-1.75 mg/kg/dose PO QID. Max. 100 mg/dose.
Prophylaxis: 1-2 mg/kg/dose PO QHS. Max. 100 mg/dose.
Adults:
Treatment: 50-100 mg/dose PO QID (or 100 mg/dose PO BID if using MacroBID capsules).
Prophylaxis: 50-100 mg/dose PO QHS.

Comments
Contraindicated in infants < 1 month of age.
Should not be used to treat UTIs in febrile infants and young children in whom pyelonephritis is likely.
Adequate urine concentrations may not be attained in patients with renal impairment.
Administration with food or milk may lessen GI upset.
