Dosages
Vasodilator/preload/afterload reduction :
Continuous IV Infusion: 0.5 - 10 mcg/kg/min
Topical for afterload reduction:
Dose not well established in children but can be approximated by calculating total daily IV dose and converting this amount to cm of ointment and dividing into two daily doses.
Continuous IV Infusion: 0.5 - 10 mcg/kg/min
Start at 0.1 - 1 mcg/kg/min and increase by 0.5 -1 mcg/kg/min every 3 - 5 min until desired clinical effect.
Topical for afterload reduction:
Dose not well established in children but can be approximated by calculating total daily IV dose and converting this amount to cm of ointment and dividing into two daily doses.

Standard concentrations for Critical Care:
PICU: 100 mcg/mL, 200 mcg/mL, 500 mcg/mL
Comments
Infusions in Critical Care areas only. Requires invasive hemodynamic monitoring.
Nitroglycerin infusions can cause a significant drop in BP. Use cautiously in hypovolemia.
Administer continuous infusions via a syringe pump with non-PVC tubing. Maximum concentration: 500 mcg/mL.
When converting from IV to topical: 1 cm of 2% ointment contains approximately 7.5 mg of nitroglycerin
