Acute Otitis Media:
45 mg/kg/dose PO BID or
15-20 mg/kg/dose PO TID using 7:1 formulation
High dose (see comments)*: 30 mg/kg/dose PO TID

Gastrointestinal tract decontamination in intestinal failure:
15 mg/kg/dose PO BID x 1-2 weeks using 4:1 formulation
IV formulation:
<40kg:
1 to <3 months and <4 kg:
Standard dose^: 25 mg/kg/dose IV q12h using 5:1 formulation
High dose: 50 mg/kg/dose IV q12h using 10:1 formulation
>= 3 months and >= 4 kg:
Standard dose: 25 mg/kg/dose IV q8h using 5:1 formulation (max: 1000 mg/dose)

Comments
Incidence of diarrhea is higher than with amoxicillin alone.
Daily doses of clavulanic acid over 10 mg/kg/day are linked with higher risk of excessive diarrhea
*Pneumonia - high dose recommended for patients that are not immunized, have risk factors for penicillin-resistant S. pneumonia (ie. travel history), or present with invasive infection.

Acute otitis media dose (7:1 formulation) may be achieved with the 4:1 formulation by giving 40 mg/kg/24 hr amoxicillin/clavulanic acid with 40-50 mg/kg/24 hr plain amoxicillin.
^ For infections caused by Gram-negative infections such as intra-abdominal, complicated UTI, and severe, invasive infections.
