Standard Prescription

ondansetron__mg PO Q__H (PRN) (__mg/kg/dose)

ondansetron__mg IV Q__H (PRN) (__mg/kg/dose)


ondansetron__mg/kg/hr continuous IV infusion

Dosages

Chemotherapy induced nausea and vomiting:  
IV: 0.15 mg/kg/dose (max. 8 mg/dose)  IV Q8H
Alternate dosing: 0.45 mg/kg/24 hr (max 24 mg/24 hr) by continuous IV infusion
PO:  0.2 mg/kg/dose PO (max. 8 mg/dose) Q8-12H

Post-Op Nausea and Vomiting:
0.1 mg/kg/dose IV Q8-12H PRN (usual max. 4 mg/dose)
0.15 mg/kg/dose IV Q8-12H PRN may be used for some neurosurgical procedures (max. 8 mg/dose)

Gastroenteritis:
IV: 0.15 mg/kg IV once (max. 8 mg/dose)
PO: 0.2 mg/kg/dose PO once (max. 8 mg/dose)
Alternate PO dosing:
8-15 kg: 2 mg/dose PO once
>15-30 kg: 4 mg/dose PO once
>30 kg: 8 mg/dose PO once

Antiemetic; Selective 5-HT3 receptor antagonist

tablet: 4 mg, 8 mg
quick dissolve tablets:  4 mg, 8 mg (ODT tablets)
oral solution: 0.8 mg/mL
injection: 2 mg/mL

Comments

Limited data for infants less than 6 months for the treatment of gastroenteritis.

Quick dissolve tablets disintegrate in mouth in 10-15 seconds. Absorption does not occur via the oral mucosa.

Give oral doses 30-60 min. prior to start of chemotherapy.
