Dosages
Methanol, ethylene glycol intoxication
**Dilute to 10% ethanol (v/v). Doses expressed as a 10% ethanol solution.
Loading Dose: 7.5 mL/kg/dose IV over 60 minutes.
Maintenance: 1 mL/kg/h continuous IV infusion.
Dose adjustment required for hemodialys.
Mechanism of Action
Antidote; competitive inhibitor of alcohol dehydrogenase, preventing formation of toxic ethylene glycol and methanol metabolites.
Forms Supplied
Injection: 99% ethanol; 10 mL ampoules.
Injection for CVC Lock: 70% (prepared by Pharmacy in 5 mL syringe)
Comments
For methanol or ethylene glycol intoxication, consult DPIC and refer to DPIC Poison Management Manual. Fomepizole is the preferred antidote.
Titrate to a therapeutic level of 22-32 mmol/L (100-150 mg/dL).
Possible adverse effects include: tiredness, headaches, dizziness, nausea, light-headedness.
Drug interaction with metronidazole. Monitor for flushing, palpitations, tachycardia, nausea, vomiting.
