Standard Prescription

famotidine   mg IV Q   H (   mg/kg/dose)

Dosages

GERD:
Infants (term to 3 mos):
0.25 - 0.5 mg/kg/dose IV Q24H
0.5 mg/kg/dose PO once daily. Max: 1 mg/kg/dose

Children >=3 mos to 1 year:
0.25 - 0.5 mg/kg/dose IV Q24H. Max: 20 mg/dose
May increase dose to Q12H.  
0.5 mg/kg/dose PO BID. Max: 1 mg/kg/dose

Children >1 year of age:
0.25 - 0.5 mg/kg/dose IV Q12H. Max: 20 mg/dose
0.5 mg/kg/dose PO BID. Max: 1 mg/kg/dose; Max: 40 mg/dose

Adults (>=16 yrs):
10-20 mg PO BID

Stress Ulcer Prophylaxis in ICU:
0.5 - 1 mg/kg/dose IV Q12H. Max: 20 mg/dose

Tablets: 10 mg, 20 mg, 40 mg
Suspension (BCCH): 8 mg/mL

Comments

Precautions: Reduce dose in patients with renal impairment.

May lengthen QTc interval.

There is no data regarding the administration of famotidine continuously to infants or children.
