penicillin G benzathine __units IM ___(frequency) (__units/kg/dose)
Dosages
Syphilis:
Primary, secondary and early latent (<1 year duration): 50,000 units/kg IM once.
Late latent (>1 yr duration) or latent syphilis of unknown duration: 50,000 units/kg/dose IM weekly x 3 weeks.
Max dose: 2.4 million units/dose.
Rheumatic fever - secondary prevention :
< 27 kg : 600,000 units IM every 3-4 weeks.
> 27 kg : 1.2 million units IM every 3-4 weeks.

Mechanism of Action
Beta-lactam antibiotic.

Injection: 1,200,000 IU/2 mL PLS
Comments
Deep IM injection ONLY.
Do not give IV; intravenous administration has been associated with cardiopulmonary arrest and death.
Do not inject into or near artery or nerve (may result in neurovascular and neurological damage).
Not a substitute for IV penicillin G; used in certain situations only; consult ID service.
Warming to room temperature will lessen pain on injection.
Provides sustained levels for 2-4 weeks.
