Acute Otitis Media:
45 mg/kg/dose PO BID or
Pneumonia:
Standard dose: 15 mg/kg/dose PO TID
High Dose*: 30 mg/kg/dose PO TID
Maximum: 4 g/24hr
Adults:
Usual dose 500-1000 mg PO TID
>5 yrs: 250 mg PO BID
Standard dose: 8-16 mg/kg/dose PO TID

15-20mg/kg/dose PO TID
Standard dose: 15 mg/kg/dose PO TID
High Dose*: 30 mg/kg/dose PO TID
Invasive, Non-AOM Infections and Cystic Fibrosis:

Comments
Achieves serum levels about twice those of oral ampicillin
*Pneumonia - high dose recommended for patients that are unimmunized, have risk factors for penicillin-resistant S. pneumoniae (ie. travel history), or present with invasive infection
