Standard Prescription
penicillin V __mg PO Q__H (__mg/kg/24 hr)
Dosages
Children:
8-17 mg/kg/dose PO Q8H OR 6-13 mg/kg/dose PO Q6H
Max: 2 g/day
Prophylaxis for Asplenia:
> 3 months to 5 years old: 150 mg PO BID.
>5 years old: 300 mg PO BID.

Beta-lactam antibiotic.

Comments
Oral solution no longer available. Tablets can be split. Pharmacy will round dose to nearest available tablet strength.
1600 U Pen G = 1 mg Pen V.
Take on an empty stomach (1 hr before or 2 hrs after meals).
For asplenia prophylaxis in infants and that cannot swallow tablets, use amoxicillin oral suspension.
