Standard Prescription

ziprasidone__mg PO__(frequency)

Dosages

Initial dose:
Children/Adolescents: 10 to 20 mg PO BID
Increment:
Increase total daily dose by 20 to 40 mg every 3 days
Usual dose range:
Children: 40 to 100 mg/day
Adolescents: 80 to 160 mg/day

Mechanism of Action

Second-generation (atypical) antipsychotic (SGA)

capsule:
20 mg, 40 mg, 60 mg, 80 mg

For doses below 20 mg: Dissolve contents of 20 mg capsule in 10 mL simple syrup to give 2 mg/mL suspension (stable x 24 hrs only)

Comments

Second-generation antipsychotic (SGA). Not effective in randomized controlled trial for treatment of schizophrenia in adolescents. Effective for treatment of bipolar disorder.

Must be administered BID in divided doses.

Give with meals (>500 calories) to improve absorption and decrease GI upset.

Risk of QTc interval prolongation: contraindicated with other drugs that have QTc prolongation as a labeled warning or precaution. An ECG is recommended in children before starting and at target dose.

Common adverse effects: sedation, extrapyramidal symptoms, orthostatic hypotension.

Risk of metabolic abnormalities (including weight gain, hyperglycemia and lipid abnormalities) present but lower than other SGAs. Must do baseline & periodic assessment of height, weight, waist circumference, blood pressure, fasting glucose & lipid profile. Refer to www.camesaguideline.org for more information.
