Dosages
Children:
6 mo to <5 yrs: 10 mg/kg/dose PO Q12H
>=5 yrs: 10 mg/kg/dose PO once daily
Maximum: 750 mg/day
Adults:
500-750 mg PO once daily
IV: same dosage as PO

Mechanism of Action
Fluoroquinolone antibiotic; inhibits bacterial DNA synthesis.

tablet: 250 mg, 500 mg, 750 mg
oral suspension (BCCH): 50 mg/mL
IV solution: 5 mg/mL in 5% dextrose injection
oral suspension (BCCH): 50 mg/mL
IV solution: 5 mg/mL in 5% dextrose injection
Comments
Oral bio-availability = 99%.
Avoid concomitant administration with antacids, sucralfate, ferrous sulfate, calcium supplements; space doses by 2-4 hours.
Dosage adjustment required in renal impairment.
