Mechanism of Action
Cefepime is a fourth-generation cephalosporin antibiotic. Cefepime has an extended spectrum of activity against Gram-positive and Gram-negative bacteria, with greater activity against both types of organism than third-generation agents.

Consult local guidelines for empiric therapy of choice for fever and neutropenia
The currently recommended dose for fever and neutropenia is higher than other references. This dose is recommended to target an MIC of 8 mcg/mL, which is adequate for most Gram negative organisms. An alternate antibiotic may be required for treating Pseudomonas aeruginosa infection, depending on the MIC. ID consult recommended.
Dose adjustment required in renal impairment.
