daptomycin __ mg IV Q _ H (__ mg/kg/24hr)

Dosages

MRSA Bacteremia and Osteomyelitis:

<=6 years: 12 mg/kg/dose IV Q24H

7 - 11 years: 9 mg/kg/dose IV Q24H

12 - 17 years: 7 mg/kg/dose IV Q24H

Mechanism of Action

Lipopeptide antibiotic, disrupting bacterial cell membrane function

Injection: 500 mg vial

Comments

Restricted to treatment of severe infections caused by resistant Gram-positive organisms, where other agents cannot be used.

Monitor creatine phosphokinase at baseline and once weekly during therapy.
