Dosages

Turbuhaler:
Low dose:
100-200 mcg/dose inhaled BID
Medium dose:
>200-400 mcg/dose inhaled BID
High dose:
>400-800 mcg/dose inhaled BID

Respiratory Solution:
Low dose:
0.125-0.25 mg/dose inhaled BID
Medium dose:
>0.25-0.5 mg/dose inhaled BID
High dose:
>0.5-1 mg/dose inhaled BID

Mechanism of Action

Inhaled corticosteroid

Forms Supplied

"Turbuhaler" inhaler: 
100 mcg/inhalation, 200 mcg/inhalation
(400 mcg/inhalation turbuhaler is non-formulary at BCCH)

Respiratory solution:

0.25 mg/mL - 2 mL nebule
0.5 mg/mL - 2 mL nebule

Comments

Not available as MDI (metered dose inhaler) puffer. Refer to fluticasone, ciclesonide, or beclomethasone monograph for inhaled steroid MDI dosing.

Significant improvements may be seen within 4-7 days.  

Doses for different administration methods (nebulizer vs turbuhaler) are not interchangeable

Dose depends upon severity and response.  Titrate to lowest effective dose.

High dose may be associated with significant side effects → refer to asthma specialist.

Side effects: dysphonia and oral thrush.  Rinse mouth after inhalation. 
 
100 mcg ciclesonide= 125 mcg fluticasone = 100 mcg beclomethasone dipropionate = 200 mcg budesonide

Health Canada approved age of use for Turbuhaler is >6 years old.

May dilute respiratory solution in 2-4 mL of NS and administer via nebulizer. May be mixed with salbutamol respiratory solution.
