Dosages
Infants and Children:
Max 12 g/24 hr
Mild to Moderate Infections: 17-35 mg/kg/dose IV Q8H.
Severe Infections: 35 - 50 mg/kg/dose IV Q8H (Max 2 g/dose).
Osteomyelitis and Cystic Fibrosis: 37.5 mg/kg/dose IV Q6H (max 2 g/dose)
Adults:
Usual dose 1-2 g IV Q8H.Max 12 g/24 hr

Does not enter CSF.
NOTE: For enteric Gram negative organisms, cefazolin susceptibility does not indicate susceptibility to cephalexin.
