Standard Prescription

chloramphenicol __mg IV Q    H (__mg/kg/24h)

chloramphenicol 0.5% eye drops- Instill one drop in__eye Q__H

chloramphenicol 1% eye ointment- Apply to __eye Q__H

Dosages

Meningitis:  
18.75-25 mg/kg/dose IV Q6H
Max: 1 g/dose
Other infections:
12.5-25 mg/kg/dose IV  Q6H
Max: 1000 mg/dose

Mechanism of Action

Bacteriostatic antibiotic, inhibiting protein synthesis, preventing protein chain elongation by inhibiting the peptidyl transferase activity of the bacterial ribosome.

Comments

Serious blood dyscrasias have occurred with short and long-term use. Monitoring CBC during therapy is recommended.

Therapeutic Peak Levels: 15-25 mg/L. Levels > 50 mg/L are associated with a greater incidence of toxic effects.

Measurement of serum levels is subject to approval of a clinical microbiologist.

Use with caution in G6PD deficiency, renal or hepatic dysfunction and in neonates.
